What does CHAP do?

CHAP connects families with medical homes in their communities. Unlike Emergency Rooms or walk-in clinics, a medical home is a doctor’s office or community clinic that serves as a home base to provide the wellness care children need. By connecting Michigan’s families to these medical homes, CHAP ensures our children are healthy and stay healthy.
