Kent CHAP assists families in navigating the healthcare system and maintaining the health of their children. Kent CHAP provides case management services, connections to community resources, and addresses social determinants of health (SDoH).
Kent CHAP can help with:
Assistance in finding a child’s medical and dental home | In-home asthma and COPD education | Help with free transportation to qualifying medical and dental home visits for CHAP eligible children | Home visits from Community Health Workers, Social Workers or Nurses to give extra help finding and navigating community resources and healthcare services | Links to counseling and other social work services
Who is eligible?
Kent CHAP is open to children from birth to age 17 who have Priority Health Medicaid, Molina Medicaid, or are uninsured.
