WCHAP is committed to improving the health of the children in Detroit and Wayne County by helping parents and their children get the most out of their relationship with their pediatrician or family physician. WCHAP provides health education to families and acts as a voice for issues concerning children's health. WCHAP has a multi-disciplinary team who work with doctors to refer families for a variety of reasons including well-child exams, lead screening, immunizations, and assisting parents with barriers to keeping their doctor appointments. Our services include support for preventative healthcare; asthma education which includes home visits; early hearing screening and Fit Kids 360, an 8 week healthy lifestyles program children 5-17 and their families, who are struggling with obesity.
WCHAP works with 8 health plans and many primary care physicians to help their members understand and recieve health care. They are integrated into a large multidisciplinary system in Detroit - Wayne County which is the largest county in the state of Michigan.
Who is eligible?
Children from ages birth to 17 who qualify for Medicaid health insurance.
